AbstrakLatar belakang : Bakteri penghasil Extended-Spectrum Beta-Lactamases (ESBL) merupakan bakteri yang resiten terhadap banyak antibiotik. Kajian Antimicrobial Resistance nasional di 8 rumah sakit pada tahun 2016 dengan ESBL pada Escherichia coli dan Klebsiella pneumoniae sebagai indikatornya mendapatkan hasil prevalensi sebesar 60%. Infeksi campuran bakteri pada pasien otomikosis merupakan sebuah tantangan dan masalah bagi dokter spesialis Telinga Hidung Tenggorokan maupun pasien. Tujuan : Penelitian ini betujuan untuk untuk menentukan prevalensi infeksi campuran bakteri ESBL pada pasien dengan diagnosis klinis otomikosis. Metode : Penelitian ini merupakan penelitian deskriptif dengan pendekatan cross sectional. Penelitian ini dilaksanakan pada bulan Juli – Oktober 2022 di Klinik THT-KL RSUD Prof. Margono Soekarjo Purwokerto dan Laboratorium Mikrobiologi Fakultas Kedokteran Universitas Jenderal Soedirman. Jumlah subjek dalam penelitian ini sebanyak 44 pasien yang diambil dengan metode total sampling sesuai dengan kriteria inklusi yaitu pasien yang didiagnosis klinis otomikosis dan berusia > 17 tahun. Penelitian dilakukan dengan cara sampel sekret telinga pasien diambil oleh dokter spesialis THT, kemudian langsung diinokulasikan pada HiCromeTM UTI Agar M1353. Bakteri penghasil ESBL diamati dengan melakukan subkultur isolat gram negatif yang tumbuh di media HiCromeTM UTI Agar M1353 ke dalam media HiCrome™ ESBL Agar. Hasil : Subjek penelitian paling banyak berada pada rentang usia 26 - 35 tahun (28,57%), dengan persentase jenis kelamin pada pasien otomikosis seimbang antara laki- laki dan perempuan sebesar 50%. Dari total 46 sampel, didapatkan prevalensi infeksi campuran oleh bakteri ESBL pada pasien dengan diagnosis klinis otomikosis sebesar 4,34%. Bakteri ESBL yang ditemukan adalah Pseudomonas aeruginosa. Kesimpulan : Prevalensi infeksi campuran bakteri ESBL pada pasien dengan diagnosis klinis otomikosis sebesar 4,34% dan bakteri yang ditemukan adalah Pseudomonas aeruginosa.
Abstrak (Inggris)Introduction : Extended-Spectrum Beta-Lactamase (ESBL)-producing bacteria are resistant to many antibiotics. In Indonesia, a National Antimicrobial Resistance study in 8 hospitals in 2016 with ESBL on Escherichia coli, and Klebsiella pneumoniae as indicators showed that the prevalence was 60%. Mixed bacterial infection in otomycosis patients is challenging and a problem for Ear Nose Throat professionals and patients. Objective : This study aims to determine the prevalence of mixed ESBL bacterial infection in patients with a clinical diagnosis of otomycosis. Methods : This research is a descriptive study with a cross-sectional approach. This research was carried out from July to October 2022 at the ENT Clinic, Prof. Margono Soekarjo Purwokerto Hospital and Microbiology Laboratory, Faculty of Medicine, Jenderal Soedirman University. The number of subjects in this study was 44 patients taken by the total sampling method according to the inclusion criteria. These namely patients were clinically diagnosed with otomycosis and older than 17 years.The study was conducted by taking samples of the patient's ear secretions from an ENT specialist, followed by inoculating the samples with HiCromeTM UTI Agar M1353. Gram-negative isolates grown in HiCromeTM UTI Agar M1353 media were subcultured into HiCromeTM ESBL Agar media to detect ESBL-producing bacteria. Results : . The bulk of research participants (28.57%) were between the ages of 26 and 35; male and female otomycosis patients were equally represented at 50% each. The prevalence of mixed infection with ESBL bacteria in patients with a clinical diagnosis of otomycosis was 4.34% from a total of 46 samples. Pseudomonas aeruginosa was the ESBL bacteria discovered.
